Personal Funds Only: This contribution is made from my own personal funds, and not those of a corporation, labor union, or national bank.

U.S. Citizenship: I am a U.S. citizen or a lawfully admitted permanent resident (i.e., green card holder).

Age Requirement: I am at least 18 years old.

No Reimbursement: This contribution is made knowingly and voluntarily from my own funds, and I will not be reimbursed by another person or entity for this contribution.
